Tuition Assistance Application
"form 9416"
Purpose; The Tuition Assistance Application is used to request reimbursement
for approved class costs or to request payment authorization validation required
to register for a class.
Tuition Assistance Registration
Authorization "form 9417"
Purpose; The Registration Authorization form is used to request authorization
to register for a course at a participating institution, where payment for
the course will occur directly from the City of Detroit tuition assistance
program, if the student receives a satisfactory grade in that course for the
time frame of the registration.
Tuition Assistance Return "form
9418"
Purpose; The Tuition Assistance Return form is used to inform the Employee
Services Specialist (at Employee Services) and the employee that additional
information is required to process a Tuition Assistance Application.
